You may wish to ask different questions of your donors depending on the level at which they donate. To do so, you will want to attach a Form to your Donor Levels. To do so, you will first want to build your custom form.

Once you have that Form created, navigate to **Fundraising** --> **Levels** --> **Edit**. Scroll to the section titled "Other Options". Next to "Form", there will be a dropdown menu of all of the forms you've created. Select the desired form and click **Save Changes**.
