You may wish to ask additional questions of purchasers if they select one of the Add-Ons. To accomplish this, you will want to attach a Form to the Add-On. Just like attaching Forms to other areas within Ludus, this is incredibly simple.

Locate the event/collection the Add-Ons are attached to.

Next to the title, click **Pricing** --> **Add-Ons**.

Click **Edit** next to the desired Add-On.

Scroll to the bottom of the page and click **Advanced Options**.

For the section titled **Form**, choose the desired Form.
