The Student Registration Form is presented to patrons when signing up for **Classes**. The Student Registration Form starts with ten default questions: First Name, Last Name, Email, Date of Birth, Allergies, Accommodations, Medications, Photo Permission, Comments, and Grade. In addition to the default questions, you can add your own custom questions to ask of registrants. Settings can be chosen for any question to control if the question is required and of whom it is asked (Adult registration fees versus Under 18 registration fees
