Attach a Form to a Class fee
Forms are found throughout Ludus and are a way to collect additional information from purchasers beyond name, email, phone number, and address. To learn more about Forms and the various fields you can set up, check out this article. Suggested Article: Forms

While you can attach a Form to a Class overall, there might be times when you only want to ask the questions on a form if they choose a specific fee. (i.e. You have a Pre-K price and an elementary price and you only want to ask special questions for people who pick the Pre-K price and not the elementary price). Check out the steps below to learn how to achieve this.

Before going to attach it, make sure you have built your Form. To learn how to do this, check out our article. Suggested Article: Create a Form

From the top navigation bar, click Collections--> Classes.
Next to the desired Class, click Pricing --> Fees.
Find the fee to which you'd like to attach a form and click Edit.
Click Advanced Options and then scroll down to the section titled "Form"
From the drop-down menu, select the form you'd like to attach.
Click Save Changes.

If there are any forms attached to the Class as a whole, patrons will also be asked to fill those out during checkout. Registrants will also have to fill out questions on the Student Profiles.
